Our membership-based model allows us to provide exceptional relationship-based care, with timely, unhurried visits, coordinating care to support you through every stage and transition of life.

We are a direct primary care practice, and do not bill insurance. Instead, your care is covered through a monthly membership fee that gives you personalized attention, integrative services, transparent pricing, discreet prescribing, and a trusted partner in your wellness journey.
Wellness Assessment
Your first visit will be a comprehensive whole health wellness or divorce wellness assessment. This may be a stand-alone visit, or serve as your intake into the monthly membership:

Initial Wellness Assessment – $300
On request, we will provide a superbill for you to submit to your insurance company for the initial wellness visits.

Holistic Primary Care
Personalized holistic primary care, including mental health, physical health and women's care; wellness resources for somatic healing including mindfulness, breathwork and movement practices.
$150/month
Lifestyle Management
Holistic primary care, plus monthly motivational coaching for lifestyle change.
$250/month
All plans include secure messaging and timely, unhurried appointments when you need them, which can be scheduled the same or next day for urgent concerns.
Membership fees do not cover any care received outside of the medical office, such as emergency rooms, hospitalizations or specialty care. Medications, labs and imaging incur additional costs. Please maintain insurance or health share plan membership for these and catastrophic life events.
This practice does not accept Medicare.
You will be billed monthly, retroactive to the first of the month that you join.
