Becoming a Patient with Dr. Kohar Jones

• Begin by completing the Contact Us form on the website.
  You’ll be asked to provide your email and phone number, and there’s space to briefly share anything you’d like Dr. Jones to know.

• After you submit the form, Dr. Jones will personally reach out to you to schedule a free initial call.

​

• You’ll have a brief conversation by phone or on zoom to explore whether the practice is a good fit for your needs.

​

• If it feels like a good match, you’ll be registered to join the practice, scheduled for an initial intake appointment, and granted access to the secure patient portal.  

​

• On the Evolve patient portal you will complete your registration, pay your intake fee, and fill out your intake health questionnaires. The portal will be your direct link to Dr. Jones, and where your health information. 

​

• Intake visits are scheduled for up to 2 hours to ensure ample time to review your health history within the total context of your life. Follow-up visits are typically 60 minutes, and shorter targeted visits (for acute needs) are 30 minutes.

​

• For concerns during business hours, patients are encouraged to reach out via portal. For urgent issues, please call the practice number to obtain timely care.

​

• Non-urgent messages will be answered within two business days.

​

• Once you join, you’ll receive a Patient Agreement that covers everything you need to know about ongoing care, communication, and what to expect from your experience.
