Care is provided through a membership-based model. Patients pay a monthly fee, which includes direct access to Dr. Jones, longer appointments, and ongoing wellness support. This model allows for more time, more connection, and a more personalized experience.

The monthly membership fee ranges from $250 to $350 per month, depending on the level of care and support you choose.

An intake appointment is required to join the practice, with a one-time fee of $300 with an optional additional Ayurvedic assessment for a total fee of $450.

No. This is a direct primary care model. Evolve Integrative Medicine does not bill insurance and is not enrolled in Medicare.

It is recommended that you maintain outside insurance for emergencies, hospital care, and specialty visits.

Yes, but you'll need to sign a waiver acknowledging that Evolve Integrative Medicine has opted out of Medicare. You agree not to submit any claims to Medicare for services provided here. All care is paid directly by you.

Depending on the track you choose, your membership includes:

Rare after-hours concerns will be addressed in real time by a medical professional. Most of these concerns would be best addressed by going to an emergency room.

Yes. Before your first visit, you’ll receive a Patient Agreement outlining how the practice works, including fees, communication expectations, and your rights as a patient. You can discontinue your membership at any time with 30 days’ notice, but would need to pay for missed months if you wish to be seen again.
