What kind of care does Dr. Kohar Jones, MD offer?
Dr. Jones is a family physician who provides personalized, holistic primary care grounded in your life context, for whole person health and emotional well-being. This includes treatment for common medical and mental health concerns, acute and chronic conditions, preventive care, and women's care. She supports lifestyle medicine, integrating the ancient healing practices of Ayurveda with evidence based biomedicine. She practices mind body medicine and shares tools of somatic healing for nervous system regulation and emotional resilience.

Dr Jones specializes in mind body medicine for divorce wellness, providing guided therapeutic exercises, movement, breathwork, and medications to support women in surviving and thriving through divorce and beyond.
How is care delivered?
Care is provided directly by Dr. Jones through a monthly membership-based model. Patients pay a monthly fee to access exceptional, relationship based primary care, with direct access to the doctor; timely, unhurried visits; integrated somatic healing for mind body medicine; and coordination of care across specialists and healing modalities. This model allows for more time, more connection, and a more personalized experience.
How much does it cost to be a member?
The monthly membership fee is $250 or $400 per month, depending on the level of care and support you choose.

An initial medical wellness or divorce wellness visit with Dr Jones guides the care plan, for a one-time fee of $300.
Are there different membership options?
Yes. Patients can choose from different membership tracks based on their needs:

Does Evolve Integrative Medicine accept insurance?
No. This is a direct primary care model. Evolve Integrative Medicine does not bill insurance and is not enrolled in Medicare.
The membership fees are HSA and FSA eligible.
A superbill may be provided for the initial wellness visit for you to submit to insurance.

It is recommended that you maintain outside insurance or participate in a health share plan such as Zion Health for emergencies, hospital care, and specialty visits.
If I have Medicare, can I still join?
At this time, we do not take Medicare patients. Please contact us if you would like to be placed on our wait list.

Services such as labs, imaging, medications, and care from affiliated specialists are not included in the membership fee.
How do I communicate with Dr. Jones?
Most communication is through the secure patient portal during business hours.
For non-urgent needs, messages are returned within two business days.
For urgent concerns during business hours, contact via portal → text → phone.
Rare after-hours concerns will be addressed in real time by a medical professional. Most of these concerns would be best addressed by going to an emergency room.
Is there a contract or agreement?
Yes. Before your first visit, you’ll receive a Patient Agreement outlining how the practice works, including fees, communication expectations, and your rights as a patient. You can discontinue your membership at any time with 30 days’ notice, but would need to pay for missed months if you wish to rejoin.
